New Patient Exam - $75/Adult, $50/child (0-17 or high school)

• One time only charge, at your very first visit.
• This fee will be in addition to treatment fees which are outlined below.

Re-Exam/Re-establish - $50

• This is for patients who were previously treated in office but have not been seen in 6+ months. An exam will be performed like it’s your initial appointment to re-evaluate patient’s condition.
• This fee will be in addition to treatment fees which are outlined below.

Adult Daily Visit - $50

• 18+ years old
• This fee is in addition to the new patient exam fee on your first visit, for a total of $125 on your first visit.

Child Daily Visit - $35

• 0-17 years old or still in high school
• This fee is in addition to the new patient exam fee on your first visit, for a total of $85 on your first visit.

Military, Teachers, First Responders & FULL Time Students

• Daily visits are offered at a time-of-service discount of 10%
• Initial exam fee still applies at a one-time charge of $75 for a total of $120 for your first appointment.

Family Bundle

• A time-of-service discount of -$10/patient can be applied if parent(s) and child(ren) are treated on the same day. Discount does not apply for new patient fee.

• Daily visit for 1 parent + 1 child = $65
• $40 + $25
• Daily visit for 2 Parents + 1 child = $105
• $40 + $40 + $25
• Each additional child = $25
